ワンポイント / Diving Tips
You need to answer to our Medical questionaire documents before you take entry level course, and if you have more than 1 “YES”, or if you are older then 45 years old and you smoke or you drink alcohol then you need to go to see doctor for check up and get doctor’s signature please. you can download medical questionare here.
病歴診断書の質問に１つでも該当する項目がある方、４５歳以上でお酒を飲むか、たばこを吸う人は、必ず事前に医師の了解を得て頂く必要があります。病歴診断書はこちらからダウンロードできます。
